Memory Enhancement
Delivers a continuum of services and resources — such as counseling, consumable medical supplies, caregiver respite and adult day care programs — tailored and adjusted to the specific and changing needs of clients and their families. Aging True also specializes in Alzheimer’s behavioral support services.
Florida Self–Directed Care
An innovative service delivery program placing individuals with mental health and/or substance abuse problems at the center of the decision making that affects them. Individual budgets allow participants to choose and self direct services based on their individual recovery needs. Aging True is the Florida Self-Directed Care program administrator, on behalf of the Florida Department of Children & Families, in Circuit 4 covering the counties of Clay, Duval and Nassau.
