Senior Home Health Agency
Compassionate and competent health professionals help our clients reach their health care goals — from the safety and comfort of their own homes — by coordinating with clients and their physicians to develop a strategic plan for in-home treatment. Home health services include physical, occupational and speech therapy, skilled nursing and convenient visits from a medical social worker or home health aide.
Emergency Alert Response System (EARS)
Our two-way communication devices can instantly summon medical assistance 24/7, helping clients feel safe and secure while maintaining an independent lifestyle. These devices help clients to contact emergency assistance as well as notify families when the client is in need.
Daily Money Management
Seniors in financial crisis can receive budgeting and daily money management, financial education, information on available resources and advocacy in applying for eligible benefits. Aging True also can be granted the use of Representative Payee through Social Security, Durable Power of Attorney and Court Appointed Guardianship to assist in financial management.
Personal and In Home Care
Our programs offer assistance with bathing, dressing and grooming, light housekeeping and seasonal cleaning, simple household repairs, laundry, grocery shopping, lawn care, pest control, and more.
Community Care for the Elderly
Through our established network of partners, Aging True offers a continuum of care designed to provide effective support for frail and elderly clients. Case managers maintain a close relationship with each client and supervise services such as personal care, homemaking, in-home services, caregiver respite, adult day care, home-delivered meals and emergency alert response systems.
Transitional Care ensures a safe and successful transition from one care setting to another. Transitional Care provides convenient in-home care at no cost for as long as is authorized (typically for up to 90 days) for individuals 60 years and older who are leaving a hospital, nursing facility, or other care site. The goal is for individuals to continue recuperation at home, preventing re-hospitalization.
