This full time position is responsible for courteous, safe and timely delivery of all food and supplies to nutrition sites and individual client homes as designated by daily routes within Duval and Clay County. This position will also assist with the routine cleaning and maintenance of the service vehicles. The candidate must successfully complete all pre-hire screenings, including a Level II criminal background screen and drug screen. The pay rate for this FT position is $15.00/hr.
The IT Associate works closely with the IT Manager to meet the IT needs of our organization. This is a full-time, non exempt position with an hourly rate of $22.00/hour. The IT Associate position plays a key role ensuring that our agency operates with the highest quality IT services available to all employees of our agency.
HHA/In Home Homemaking: Position entails the assisting of functionally impaired adults with the provision of home care services. These services will include but are not limited to bathing, dressing, assist with ambulating, security, eating, household and homemaking supervision, meal prep (if necessary) errands and reminder of self administered medications. Services are provided in both Duval and Clay Counties. Ability to work in Clay County is a plus.
Helping to maintain people’s independence is the cornerstone philosophy of Meals on Wheels. For forty years we have been providing hot, nutritious meals to home-bound elderly and disabled persons in Duval County and now Clay County. This position will be directly responsible for referrals regarding potential clients. Primary functions are also to include outreach, client eligibility/assessments, client education regarding funding, file maintenance, client counseling, nutrition education, and any other duties needed to support the daily operations of the Meals on Wheels Program.
Case Managers will provide potential and existing clients with a thorough assessment of their current situation (demographics, mental and physical health, nutrition status, health conditions, caregiver status, social resources, and environment). In addition, Case Manager will interact with staff within Aging True and throughout the Foundation, elders and their loved ones, and service providers throughout the community.
The Case Management Intake Specialist is the central position through which all customer and constituency inquires are coordinated for our Home and Community-based programs in Clay and Duval Counties. This position interacts with all other departments continuously to provide service solutions. This position will report to the Director of Care Services and must have a thorough understanding of all essential operational elements of the organization. The representative will also receive, document, and refer to community resources.
The SDC/Mental Wellness Office Support Specialist will be instrumental in providing administrative support to the Director of Mental Wellness and other key management staff. This individual will interact with staff from throughout the Mental Wellness Division, support staff, elders and their loved ones, and service providers throughout the community.
